Ardent Health has partnered with Bravo to administer and engaging wellness program that produces real results.
Participation in the program is voluntary and confidential, but by taking part you will have the opportunity to earn significant rewards while making strides to a healthier you.
How it Works.
If this is your first time using this portal, select Create an Account. This will allow you to create your account and begin the registration process. You must have a verified account before you will be able to gains access. Please be sure to check your email to complete the verification step. To re-enter the portal after creating an account, select Login and enter your Username and Password. Helpful links are available to retrieve your information, if needed.
Complete your Health Assessment You will need to answer a few questions about your health and lifestyle. The questionnaire answers along with the biometric results will generate a report that highlights information regarding your current health status, areas of risk and suggestions on how to improve your health
Schedule your wellness appointment*
Follow the step-by-step instructions to schedule your onsite screening appointment. You can also opt to download an approved provider screening form or LabCorp voucher to visit your own provider or approved LabCorp clinic.
*Participants who choose to screen using the provider form may be responsible for the cost of their screening if they don’t have health coverage, or if services are not covered under their current health plan.
To qualify for the incentive, new hires employees and their spouse/domestic partner must register and complete the screening within 60 days of the employee’s hire date.
Re-hires and their spouse/domestic partner must register and complete the screening within 60 days of the employee’s re-hire date.
Newly eligible employees and their spouse/domestic partner must register and complete the screening within 60 days of when you became benefits eligible.
Employees and their spouses will each need to create or login to their own separate accounts to maintain individual privacy, and must schedule their own individual appointments.
ADDRESS CHANGES - ALL EMPLOYEES: All result letters will be available on the website, and one letter will be sent by mail to the address provided by your employer. If you have had an address change and the address on your registration is incorrect, you must notify your employer to have the address changed in order for the change to appear in Bravo's system. You will be able to view your results and/or results letter online with a validated account, approximately 10 business days after your screening event, at www.bravowell.com/ardent.
