Employees and their spouses will each create or login to their own separate accounts to maintain individual privacy, and must schedule their own individual appointments. Just follow the instructions below:
If this is your first time using this portal, Select Create an Account This will allow you to create your account and begin the registration process.
To re-enter the portal after creating an account, select Login and enter your Username and Password. Helpful links are available to retrieve your information, if needed.

Onsite New Hire Screening Event - Schedule in Step 2 of Online Registration Process.
Physician Forms to screen with a Healthcare Provider- Download from your Dashboard. NOTE: Physician Forms should only be used after medical benefits are effective. Participants who choose to screen prior to their medical effective date may be responsible for the cost of their screening if they don’t have other coverage. Previous Labs can be submitted from prior screenings back to August 1, 2017.
LabCorp Voucher at a LabCorp Patient Service Center - Download voucher and instructions from your Dashboard. NOTE: NOT AVAILABLE for St. Francis participants.

BSA New Hire Employees and their Spouses will screen at the New Hire Walk-in Clinic in the Employee Health Department on Friday mornings, or they can choose a Physician Form in the Scheduling Step #2.
ADDRESS CHANGES - ALL EMPLOYEES: All result letters will be available on the website, and one letter will be sent by mail to the address provided by your employer. If you have had an address change and the address on your registration is incorrect, you must notify your employer to have the address changed in order for the change to appear in Bravo's system. You will be able to view your results and/or results letter online with a validated account, approximately 10 business days after your screening event, at www.bravowell.com/ardent.
